What’s your main reason for bringing your dog to daycare?

My dog(s) must be in general good health and I must provide proof of current C5 vaccinations (which included a Kennel cough component) in order for my dog(s) to attend Doggy Day Care at The Dog Dazzlers. C5 Vaccination certificate can be provided by your Vet. I must inform The Dog Dazzlers if my dog has been infected by any communicable diseases.
If my dog(s) require any medication for any condition, I must supply the medications with specific written dosage instructions, in addition to this, a Medicinal authorisation form is to be signed by both parties at reception. In the event of illness, I authorise The Dog Dazzler staff to take my dog(s) to our recommended Vet: Burwood Vet (128 Highbury Road Burwood Vic 3125), if emergency treatment is required. I will pay in full any Vet expenses upon collection of my dog(s) from The Dog Dazzlers.
I must inform The Dog Dazzlers of any known behavioural problems that my dog(s) may have, including, but not limited to, excessive barking, dominance, mounting etc. The Dog Dazzlers uses safe and harmless corrective methods such as, verbal commands, crate training and timeout rooms when reprimanding a dog’s behaviour.
All dogs at The Dog Dazzlers Dog Day Care must be desexed (over the age of 8 months), be well socialised and friendly to not only other dogs but people as well. Any dog found to be aggressive will not be accommodated at The Dog Dazzlers Doggy Day Care Facility. Any behaviour deemed dangerous or inappropriate at The Dog Dazzlers Doggy Day Care Facility may result in my dog(s) removal from the facility.
All dogs must approach, enter and leave the facility on a lead held by the staff or owner at all times. All dogs must wear a collar and ID tag at all times whilst attending The Dog Dazzlers Doggy Day Care Facility.
The Dog Dazzlers Doggy Day Care Facility is a open environment that allows dogs to interact and play directly with one another off leash and under supervision. Every dog reacts differently and animals, by nature, can be unpredictable and therefore present a certain level of risk. I acknowledge and understand that there are certain risks involved in Doggy Day Care, including but not limited to people, property and animals. The Dog Dazzlers Doggy Day Care staff will administer first aid if required to all minor injuries and notify me of said injury to my dog(s) that may occur in normal activities in a Doggy Day Care environment. My dog(s) is cared for at The Dog Dazzlers Doggy Day Care facility without liability for loss or damage from disease, theft, fire, escape, injury or harm to persons, other dogs and property.
In case of emergency or for the use of The Dog Dazzlers transport service, I recognise the risks of injury that accompany said transport to and from the facility or any other necessary location.
I understand that The Dog Dazzlers Doggy Day Care Facility operating hours are 7am – 6:30pm If I do not pick up my dog(s) by 6:30 pm there will be a late fee charged for every 15 minutes or part thereof after 6:30pm
I understand that dogs must be dropped off by 11am for daycare attendance
I accept that The Dog Dazzlers fees are to be paid in advance or on drop off the morning of the scheduled visit.
I accept that I must pre-book my dog(s) attendance at The Dog Dazzlers Doggy Day Care Facility. I understand that a “no show” or cancellation after 5pm the day before the scheduled booking will incur a fee of $55.
I understand that if I have not received a confirmation SMS confirming my dogs attendance I may be refused service on the day and that walk-in bookings will not be accommodated
I hereby release The Dog Dazzlers Doggy Day Care Facility from any liability or claim of injury or loss associated with my dog(s) attendance and participation at The Dog Dazzlers Doggy Day Care Facility.
I acknowledge that all photographic images or digital recordings taken of the facility, dogs, customers and staff, together with their prints and their copyrights therein are the property of The Dog Dazzlers Doggy Day Care Facility. I authorise The Dog Dazzlers to reproduce, alter the images, in print or electronic format without any compensation to me and I waive the right to approve the finished image
I certify that I have read and understand this agreement. I accept all the terms and conditions of this agreement. I have completed The Dog Dazzlers Doggy Day Care Enrolment Form to the best of my knowledge and will notify The Dog Dazzlers immediately of any changes to the information I have provided
If my dog has any allergies, I will bring in their own food if I would like them to be involved in any enrichment/training service
